Asthma

Asthma is a disease that affects your lungs. It causes repeated episodes of wheezing, breathlessness, chest tightness, and nighttime or early morning coughing. Asthma can be controlled by taking medicine and avoiding the triggers that can cause an attack. You must also remove the triggers in your environment that can make your asthma worse.

If your child has Asthma and will require the use of an inhaler in school please have your child’s physician fill out the Asthma Action Plan (pdf)

If your child is determined  by their physician to be capable of carrying their inhaler and administering it to themselves while in school then they will need to complete the bottom of Administration of Medication Form.pdf(grades 5 - 12 only).

Both of these forms require signature of the physician and the child’s parent/guardian.  No student is to carry an inhaler in school without authorization from the physician.
