Phoenixville Transportation Forms
To access the Act 372 Form for transportation for the 21/22 school year, please go to PASD’s website at www.pasd.com. From the homepage go to Departments, scroll down to Transportation, then click on Forms & Guidelines.
The Act 372 form must be completed for each child requiring transportation.
Keep in mind that if a completed form is not submitted to PASD’s transportation department, your child(ren) will not be scheduled for transportation
