Be sure we are aware of all your child’s allergies. Please have the appropriate form completed by your licensed prescriber with their signature, as well as, the parent/guardian signature. Medications should be brought to school by parent/guardian in the original container with pharmacy label.
If your child is in grades 5-12 and will be self carrying their epinephrine auto injector, please have your physician and yourself complete and sign this form:

By Law, all school children are served milk. A Doctor’s Note is Required for:
- Allergy to milk
- Lactose intolerance
